Your UCare MSHO plan includes complimentary unlimited access to Caregiver Assurance™ for you or your designated caregiver* for those that qualify. Qualifying members must be those living in the community with have a diagnosis of dementia, Multiple Sclerosis (MS), Parkinson’s or Amyotrophic Lateral Sclerosis (ALS). We’ll partner you directly with a dedicated Caregiver Assurance Advisor—a licensed social worker trained in aging and caregiving—who will help to address your top caregiving concerns and offer suggestions, informational resources, and referrals.
Advisors are available by phone Monday through Friday from 9 a.m. until 7 p.m.

**Your designated caregiver is a single individual that may take advantage of this benefit. Before receiving a referral to Caregiver Assurance, your caregiver must confirm for the care coordinator that they have authority (authorized representative or Power of Attorney status) to discuss your protected health information.
