Frequently Asked Questions

Who is eligible to use this program?

All 2022 UCare MSHO plan members with a diagnosis of dementia, Multiple Sclerosis, Parkinson’s or ALS are eligible for Caregiver Assurance. Members must be referred by their care coordinator.

Either the UCare MSHO plan member themselves or their designated caregiver—a single individual—may use this program. When calling, the designated caregiver must have the member’s plan information available, including the member Name, UCare ID and Date of Birth.

What if I don’t meet the MSHO program eligibility requirements?

Speak with your UCare MSHO Care Coordinator to determine available options.

If you don’t qualify for this program through your 2022 UCare benefits, you may elect to purchase M Health Fairview’s Caregiver Assurance™ monthly membership. If you elect to purchase this service, you will be charged directly for the service, UCare will not cover the cost of this service. More information about our membership program and other products can be found at www.caregiverassurance.com.

How can I get started?

Members should speak with their UCare MSHO Care Coordinator for additional information about the program. The Care Coordinator can help to determine if Caregiver Assurance is the right program for the situation and provide a soft referral to the Caregiver Advisors.

If a member does need to contact the program directly for any follow-up activity, the Caregiver Advisors can be reached at (612) 672-7996.

Are interpreter services available for non-English speaking members?

Yes! The Caregiver Advisors will help to coordinate an interpreter to join all conversations if necessary. Interpreter services will be provided by UCare.
